New Student Registration Form for 2003-2004 School
Year
Each new applicant must be interviewed by the principal. At the time
of the interview bring: completed application, birth certificate,
immunization record and, if baptized, baptism certificate. A NYC Board
of Health Examination form must be completed by a doctor before school
opens in September. For students entering grades 2 through 8, a copy of
the latest report card is required.
If your child is accepted, a seat will be reserved upon payment
of the following non-refundable fees:

August 1, 2003 to May 15, 2004. |
